If you have arranged a time to come and see us to register please bring with you the following items:

• ID - preferably a passport, with any necessary visas entitling you to work in the UK
• Proof of GDC registration or proof of a course you are attending (if applicable)
• Proof of current address (ie a bill with your name on it)
• 2 references (these can be written, or just an email or phone number will do.)
• A P45 from you previous employer if you have one

We are at 8 Percy Street off Tottenham Court Road - near the HSBC bank.  From Tottenham Court road station walk north up Tottenham Court road towards Goodge street.  Percy street is on your left.
